What the Research Tells Us
The updated IOC consensus on RED-S (Mountjoy et al., 2018) expanded the former Female Athlete Triad concept to recognize that energy deficiency impairs not just menstrual and bone health, but also metabolic rate, immunity, protein synthesis, cardiovascular function, and psychological wellbeing. Ackerman et al. (2019) demonstrated that female athletes with functional hypothalamic amenorrhea (absence of periods due to low energy availability) had 10% lower bone mineral density than eumenorrheic teammates, with a stress fracture incidence 4× higher. In rowing — where the ribcage is under repetitive loading stress — low bone density translates directly to rib stress fractures, one of the sport's most common injuries. Treatment requires increasing energy availability above 45 kcal/kg FFM/day, which typically means adding 500–800 kcal/day to current intake. Return of menstrual function is the primary recovery marker and may take 3–12 months.
