Ackerman et al. (2019) demonstrated that female athletes with functional hypothalamic amenorrhea (absence of periods due to low energy availability) had 10% lower bone mineral density than eumenorrheic teammates, with a stress fracture incidence 4× higher. In rowing — where the ribcage is under repetitive loading stress — low bone density translates directly to rib stress fractures, one of the sport's most common injuries. Treatment requires increasing energy availability above 45 kcal/kg FFM/day, which typically means adding 500–800 kcal/day to current intake. Return of menstrual function is the primary recovery marker and may take 3–12 months.
